What data we collect
Through the form on this site we collect: your name, phone number, email address if you provide it, the treatment you are enquiring about and any message you choose to add. We also record the page the enquiry came from and campaign parameters, where present.
We do not ask for health information through the website. If you need to tell us something clinical, we will cover it in consultation or by phone.
Why we use it and on what legal basis
- To handle your assessment request and contact you: consent of the data subject (art. 6.1.a GDPR).
- To send you information about the treatment you enquired about: consent of the data subject.
- To comply with applicable legal obligations: compliance with a legal obligation (art. 6.1.c GDPR).
How long we keep it
We keep contact details for a maximum of 24 months from the last interaction, unless you ask us to erase them sooner or a legal obligation requires longer retention.
Recipients
Your data is passed to the selected clinic and to the providers supplying our hosting, email and patient relationship management (CRM) services, who act as processors under a signed agreement. It is not shared with third parties for other purposes, and no international transfers outside the European Economic Area take place without the safeguards required by the GDPR.
Your rights
You may exercise your rights of access, rectification, erasure, objection, restriction of processing and portability by writing to info@crookedentalclinic.com, proving your identity. You may also withdraw your consent at any time, without affecting the lawfulness of prior processing, and lodge a complaint with the Spanish Data Protection Agency (www.aepd.es).
Security measures
We apply appropriate technical and organisational measures: encryption in transit, access control, activity logging and encrypted credential storage. We do not store your IP address in the clear.
